Coroner identifies missed opportunities in teen’s allergic reaction death

A Victorian coroner has identified multiple missed opportunities in the medical treatment of a Melbourne teenager who died after suffering a severe allergic reaction to a delivery meal. James Tsindos died in hospital in 2021 after eating a vegan meal that contained 'cashew cheese'.
